– Conduct/organize outreach to community to promote mental wellness and increase public awareness of mental health
– Conduct induction talks for Go-To-Point
– Conduct mental health screening for clients after community outreach
– Identify seniors who are at-risk/with mental health conditions and connect them with the right service for early diagnosis and treatment
– Provide basic emotional support and monitoring to clients or caregivers of persons with dementia or mental health conditions through regular home
visitations
– Provide service linkage and help clients and caregivers locate the right care and assistance within the healthcare and social services systems
– Engage key community partners such as grassroots organisations, social service agencies to raise their mental health awareness, understand
services provided by CREST and coordinate mental health training and education
– Ensure proper maintenance of case documentations
– Submit reports to external parties
– Manage a team of volunteers who may help out in outreach events and conducting activities

Technical Competencies:

- Degree Holder
- Excellent communication and interpersonal skills
- A team-player who would work well in teams

Core Competencies:

- Passionate in working with seniors
- Able to communicate in English and other languages
- Detail oriented in keeping proper documentation
- This position is on Contract basis
