CareLine is a personal care telephone service by Changi General Hospital (CGH). It provides health and social support to seniors who may be living alone or are frail. It helps these seniors to keep safe. If something untoward happens to the seniors at home, for example, if they fall or feel unwell, or if they require urgent care, they may not know what to do. They may be feeling scared or anxious. In such situations, CareLine is there to support them 24 x 7. When a client enrols with us, we start building a care relationship with them so that we can better assist them. When they call CareLine for help, we can assist to coordinate the care that they need. We can help to link them up to agencies. If they need someone to talk to, we can link them up to befrienders. Community Resource Engagement and Support Team (CREST) is a community-based programme which aims to serve as a community safety net to reach out to older persons at risk of / with mental illness (i.e. depression / dementia), to address the gaps in the services in the community for the older person and to ensure continuum of care for the older person by supporting them in both basic social and health issues. We seek to promote early recognition of the signs and symptoms of depression and dementia, to encourage early diagnosis and intervention.

Jocelyn has been in the helping profession for more than 18 years. She began her career as a Rehabilitation Officer at the Singapore Prison Service before joining the Ministry of Education (MOE) as a trained secondary school teacher. She subsequently undertook her Master degree in Educational Psychology (University College London, UK) as a recipient of the MOE postgraduate scholarship. Following that, she served as a Senior Specialist at the Psychological Services Branch of MOE and delivered a range of services to primary and secondary schools. This included providing psycho-educational assessments and consultations to parents, teachers and school leaders to help support children with learning difficulties and special educational needs. Desiring to extend her support to individuals with mental health concerns, Jocelyn subsequently left MOE to complete her doctorate in Clinical Psychology and Clinical Geropsychology at the University of Queensland (Australia) under the Endeavour Postgraduate Award. Aside from direct support of individuals with learning and psychological concerns, Jocelyn’s passion is in caregiver support. Dr Joseph Leong Jern-Yi has been working at the Institute of Mental Health (Singapore) since 2000. After obtaining his Certified Psychiatric Rehabilitation Practioner (CPRP) in 2004, he also did his clinical fellowship at UCLA with Distinguished Professor of Psychiatry Robert Paul Liberman in 2009. He was Deputy Chief of Community Psychiatry at the Institute of Mental Health working with Community Partners to build connections, capacity and capability. He shares his expertise in Psychiatric Rehabilitation and Recovery as an Expert Advisor to start up several Non-Governmental Organisations (NGOs) such Club HEAL, Caregiver Alliance, Hougang Care Center and St Andrew’s Nursing Home. He also serves on the Board of the Singapore Association for Mental Health. He is the current President for the Association for Psychiatric Rehabilitation (Singapore).
