CareLine is a personal care telephone service by Changi General Hospital (CGH). It provides health and social support to seniors who may be living alone or are frail. It helps these seniors to keep safe. If something untoward happens to the seniors at home, for example, if they fall or feel unwell, or if they require urgent care, they may not know what to do. They may be feeling scared or anxious. In such situations, CareLine is there to support them 24 x 7. When a client enrols with us, we start building a care relationship with them so that we can better assist them. When they call CareLine for help, we can assist to coordinate the care that they need. We can help to link them up to agencies. If they need someone to talk to, we can link them up to befrienders. Cost may be imposed for some of the services. For eligible clients, the service providers will advise the approximate cost of service upon assessment. Community Resource Engagement and Support Team (CREST) is a community-based programme which aims to serve as a community safety net to reach out to older persons at risk of / with mental illness (i.e. depression / dementia), to address the gaps in the services in the community for the older person and to ensure continuum of care for the older person by supporting them in both basic social and health issues. We seek to promote early recognition of the signs and symptoms of depression and dementia, to encourage early diagnosis and intervention.
