About COMIT: Community Intervention Team or, COMIT provides assessment, counselling, therapy, case management and psychoeducation support for clients with mental health issues and dementia, including their caregivers. With the assessment, the team develops individualised care plans for clients or link up the clients with other more suitable services. Caregivers will be provided with the appropriate assessment and intervention such as psychoeducation, training, support and support group. COMIT works closely with the Community Engagement Teams, General Practitioners, Polyclinics and other community partners to provide holistic care to clients. It is one of the programmes under the Community Mental Health Masterplan developed by the Agency for Integrated Care (AIC), together with the Ministry of Health (MOH). Cost may be imposed for some of the services. For eligible clients, the service providers will advise the approximate cost of service upon assessment. About COMNET: COMNET Senior Service (COMNET), abbreviated from Community Network, has its humble beginnings from the Befriending Service, initiated in 2007. COMNET’s mission is to support vulnerable and low-income seniors to live independently and actively in the community. The Community Intervention Team (COMIT) is embedded in the community to provide holistic services for clients with mental health needs and their caregivers so that they can live and age well at home and in the community. It is an allied health-led team which provides psychosocial therapeutic intervention for clients with mental health needs and supports their caregivers in coping with caregiving for the loved ones.
